Intrauterine devices (IUDs) are a popular contraceptive method because they are long-acting, highly effective and easy to use. In the United States, approximately 12% of contraceptive users choose an IUD. IUD risks include expulsion, which is most commonly seen in the first year of use, in young users, and in those with heavy menstrual bleeding. IUD expulsion rates range from 2% to 10% for all IUD users. Expulsions can be categorized as complete or partial, which occurs when the device is pulled away from the uterine fundus into the lower uterine segment or cervix where it may be associated with increased accidental pregnancy. We do not clearly understand the mechanism behind partial or complete IUD expulsions or if they may be caused by certain activities or behaviours, such as menstrual hygiene product use.
From August 2018-October 2018, the authors conducted a cross-sectional survey using two different Internet recruitment platforms. Eligible respondents were women over 18 years of age who lived in the United States. In this study, they sought to explore menstrual hygiene product use in IUD users and evaluate the relationship between products used and IUD expulsion. They found that while a majority of participants reported having used disposable pads or tampons for menstrual hygiene, a substantial proportion (19.7%) reported the use of menstrual cups. In conclusion, menstrual cup use may be higher than previously reported, particularly in IUD users. Concurrent menstrual cup use may increase the risk of IUD expulsion. Prospective research is necessary to fully explore the relationship between menstrual hygiene product use and IUD expulsions. |
